The scope is set around the client’s decision needs, regulator activity, available evidence, and the boundaries of any parallel legal or police process.
Systems and controls
The analysis considers equipment, environment, supervision, procedures, communication, workload, and other interacting factors.
Competing explanations
Relevant accounts and hypotheses are compared against the available evidence rather than accepted at face value.
Clear reporting
The report separates established facts, unresolved issues, contributing factors, and matters for the client’s decision.

- Avoid contaminating witness accounts with group reconstruction
Keep roles clear
- R6I does not replace emergency services or a regulator
- The client retains its WHS duties and corrective-action decisions
- Legal privilege must be designed with the client’s lawyer
- Any police or regulator direction takes priority
